INTRODUCTION

In the current combat realities, a number of wounded with severe injuries which require urgent surgical interventions has increased significantly. According to NATO standards, at the Role 2 level of medical evacuation, qualified surgical care is provided, which includes emergency operations to save lives, massive hemorrhage control, and stabilization of the patient’s condition before further evacuation to a higher level of medical care (Role 3).  

One of the key aspects of the effective surgical team performance is proactive supply of operating rooms with ready-to-use surgical kits. These kits may contain sterile instruments and materials for a single operation on one wounded, which significantly reduces preparation time and ensures the standardization of medical care in complex field conditions.

The structure of injuries in active combat conditions vary, but their most common types are:

-        limb and soft tissue injuries (require primary surgical wound treatment, surgical repair of major blood vessels, installations of external fixation, etc.);

-        abdominal injuries (laparotomies);

-        chest injuries (thoracotomies, thoracocentesis);

-        traumatic brain injuries (craniotomy), etc.

 

For effective medical resource arrangement, it is essential to correctly calculate the quantity and types of surgical kits, considering both, qualitative composition (surgical intervention type) and quantitative needs for a given number of wounded.

This overview provides an example of ready-to-use surgical kits distribution for Role 2, estimated for 100 wounded, which is based on battle injury statistics observed in real war conditions.

Such an approach allows for coordinated teamwork of the surgical team, reduces pre-operation time preparation, and improves the effectiveness of medical care in wartime conditions.

For 100 wounded at the Role 2 level, on average, the following ready-to-use surgical kit can be required:

  • 1 craniotomy set

  • 1 thoracotomy set

  • 5 thoracocentesis sets

  • 4 laparotomy sets

  • 3 vascular repair sets

  • 6 sets for ExFix installation

  • 5 limb amputation sets

  • 10 primary wound debridement sets (large)

  • 20 primary wound debridement sets (medium)

  • 45 primary wound debridement sets (small)

However, when using such an approach, certain challenges and inconsistencies may arise. First, it should be acknowledged that wounded localization ratio is not constant, and the given number of wounded (i.e., 100) may not often be representative in specific situations. For example, thoracotomy or craniotomy can be required only once per 300–500 wounded, while surgical repair of the soft tissue can be required for up to 70 out of 100 cases. Therefore, it is relatively difficult to determine the exact ratio of injuries by localization under such conditions. Furthermore, some features of Role 2 medical care facilities dictate that some focus on severe and moderate patients treating, while others only accumulate soldiers with light wounds.

Ceftriaxone 2 g IV one-time (or Cefazolin 2 g IV every 8 hours as an alternative). For combined abdominal wounds, add Metronidazole 500 mg IV every 8–12 hours. Duration: as per prophylactic standard - for up to 24 hours; in the presence of a CSF leak or an unclosed source of infection - as clinically indicated.

  • Additional suture materials - +1 of each type and size

  • Hemostatic agents: hemostatic sponge (e.g., Surgicel, Surgispon, etc.) — 5 pieces

  • Tourniquets (Combat Application Tourniquet) - 2–3 per kit
